Students will be registered for transportation
when registering for school. Students will be given two pre-printed forms to be
completed and signed by the parents/guardians. Both forms will be given to the bus
driver on the students' first ride. This inofmation is needed for funding purposes
and to ensure the students' safety in the event of an emergency. If a student
receives modified transportation services, the IEP Committee and the parent/guardian will
also complete the transportation form.
